Background
Project Title: UNDP/GF Project 'Continuing Scale Up of the Response to HIV with Particular Focus on Most At Risk Populations and Strengthening System and Capacity for Universal Access to HIV Prevention, Diagnosis, Treatment and Care in Uzbekistan'
In 2010 and 2011 Uzbekistan has been awarded two grants of The Global Fund to fight AIDS, tuberculosis and malaria - HIV Round 3 Rolling Continuation Channel and Round 10 grants. UNDP is a key partner to the Global Fund to fight AIDS, Tuberculosis and Malaria and works in a number of countries as a Principal Recipient including Uzbekistan. A decision by the Multisectoral Expert Council selected UNDP as the Principal Recipient for the implementation grants. In accordance with Global Fund Policy the grants have been consolidated in a Single Stream of Funding. The consolidated project is implemented by UNDP in close cooperation with governmental and non-government organizations.
One of the consolidated Project components is infection control which includes promotion of safe use and disposal of equipment in healthcare settings to prevent transmission of HIV as well as other blood borne infections such as Hepatitis B and C. In the framework of project implementation it is planned to prepare National guidelines and standards on infection safety along with Standard Operating Procedures (SOP) for effectively using and safely disposing a range of critical single use items (SUI) for invasive procedures, develop a list of essential disposable equipment and supplies, and create unified approach for their prioritization and budgeting on National, regional and facility levels for healthcare professionals involved in process.
According to the Uzbekistan National Regulation on Sanitary Epidemiological control the main structure in healthcare system responsible for nosocomial infection control and prevention is the Republican Center of State Sanitary Epidemiological Surveillance (RSSEC) and its branches in the regions. It is well organized multifunctional structure, dealing with different aspects of Public Health, Epidemiology and Hygiene issues. Currently RSSEC is one of the significant players and implementer of the current GFAMT grant on HIV in Uzbekistan.
In 2012 a new SOP on SUI needs assessment and supplement and educational module with curriculum and guideline were developed.
The main goal of present consultancy is to strengthen the capacity of the Ministry of Health of Uzbekistan to prevent the spread of HIV into the general population and to strengthen health system on Health Technology assessment to provide universal access to qualified medical services and decrease threat of HIV contamination in medical facilities in Uzbekistan.
As the continuation of the previous work an International Consultant will be hired to complete the following tasks
- To develop the template list of essential disposable equipment and supplies, and create unified approach for prioritization and budgeting on national, regional and facility levels using pharm-economical and bioethical methods together with the national experts;
- To develop manual for prioritization of SUIs and definition of the needs in SUIs for the selected types of HCFs, based on national data of morbidity and existed clinical protocols and standards;
- To develop a protocol of survey to assess the SUIs consumption in the HCFs.
Duties and Responsibilities
Objectives of Assignment
The objectives of this mission is:
- To review national guidelines, current regulation, discuss challenges and shortfalls of the current situation with the national consultants and specialists,
- To create the instruction with the unified approach for prioritization and budgeting on national, regional and facility levels based on international/WHO standards in this area.
- To assist to MoH to develop the list of essential disposable equipment and supplies, and their quantification for centralized procurement, based on created instruction
- To develop protocol with the tool on SUIs needs assessment, based on clinical standards and morbidity data for transition period.
Scope of work Expected Results/Deliverables/Final Products Expected
International Consultant working under the direct supervision of Project Manager bears responsibility to provide technical assistance in developing the list of essential disposable equipment and supplies, and create unified approach for prioritization and budgeting of SUIs on National, regional and facility levels, to develop the tool for MoH on needs assessment
First Stage – 10 days
- Desk review of available national guidelines, normative documents and regulation on disposable equipment and supplies needs assessment;
- Current system of SUIs dissemination among governmental healthcare facilities in Uzbekistan;
- To prepare a draft of the list of essential SUIs and manual on prioritization and budgeting of disposable equipment and supplies for discussion with the national specialists;
- To prepare (i) protocol and (ii) guideline to conduct a survey on SUIs consumption assessment;
- To prepare the tool and (iii) manual on SUIs needs assessment based on morbidity data and existed clinical protocols and standards.
Second Stage – 15 days
- To conduct orientation meeting and facilitate discussion with national consultants and specialists with the purpose to discuss format and details of the developed documents and to collect comments and recommendations;
- To finalize the list of essential SUIs, taking into account comments and recommendations of participants of orientation meeting;
- To submit the final version of list of essential SUIs and manual on prioritization and budgeting of disposable equipment and supplies to the Project manager of in Russian and English.
Deliverables:
- Review of available national guidelines, normative documents and regulation on single used instruments (SUIs) needs assessment, supplement and logistics in Uzbekistan;
- Instruction with unified approach for prioritization and budgeting on national, regional and facility levels based on international/WHO standards in this area;
- Draft of the list of essential SUIs, based on local information and legislation and international experience with use of unified approach for prioritization and budgeting for MoH;
- Protocol on SUIs needs assessment based on morbidity data, clinical protocols and standards, in Russian and English submitted to Project Manager;
- Report on mission in Russian and English submitted to Project Manager.
Deadline for above deliverables is February 1, 2014.
Note:
Translation of the submitted documents to Russian or English is the responsibility of the International consultant.
This is a lump sum contract includes all costs of consultancy, travel and daily subsistence allowance costs that required to produce the above-mentioned deliverables. The contract amount will be released in one instalment upon satisfactory deliverance of the deliverables and subject to acceptance by the Project Manager and upon submission of Certification of Payment as well as Performance Evaluation Form (PEF) on the works performed as indicated in this TOR for the final payment.

Required Skills and Experience
Education
- Advanced degree in Health Technology Assessment, Public Health, Medicine, Epidemiology and other relevant field
Experience
- At least 3 years' experience in the intra hospital infection control and disposable equipments needs assessment and supplement areas;
- Experience in needs assessment and supplement in the other countries, work experience in CIS countries is asset;
- Previous relevant experience in Uzbekistan is an asset.
- Knowledge of rules and procedures of UNDP will serve as an advantage.
Language Requirements
- Fluency in English, Russian is an asset.
Other:
- Availability to travel within the country, if needed.

Individual consultant will be evaluated based on the following methodologies:
Cumulative analysisThe award of the contract shall be made to the individual consultant whose offer has been evaluated and determined as:- Responsive/compliant/acceptable; and
- Having received the highest score out of a pre - determined set of weighted technical and financial criteria specific to the solicitation;
- Technical Criteria weight; [70%];
- Financial Criteria weight; [30%].
